Lähtökohdat
Parodontologisen hoidon järjestämistapojen kuvailu sekä niiden vertailu Pohjoismaiden välillä voivat auttaa terveydenhuollon suunnittelijoita ja päättäjiä meneillään olevassa kehittelytyössä, jonka tavoitteena on tehokas suun terveydenhuollon järjestelmä. Ei kuitenkaan ole olemassa yhtä kattavaa mittaria, joka kertoisi, kuinka kustannustehokkaasti hoitojärjestelmä pystyy vastaamaan väestön parodontologisen hoidon tarpeeseen.
Menetelmät
Systemaattisen kirjallisuuskatsauksen sijaan artikkeli perustuu kunkin kirjoittajan asiantuntemukseen sekä uusimpaan aihetta koskevaan tutkimustietoon. Näiden tuloksena syntyi kuvaileva katsaus parodontologisen hoidon järjestämisestä.
Tulokset
Kaikissa Pohjoismaissa tutkimusnäyttöön perustuvaa parodontologista hoitoa on tarjolla sekä yksityisellä että julkisella puolella, ja hoidon kustannuksista maksettavat korvaukset vaihtelevat 0 prosentista 70 prosenttiin. Erityisryhmiin kuuluville potilaille on tarjolla kattavampaa hoitoa, minkä mahdollistavat erityistukijärjestelmät sekä hoidon tarjoaminen yksityispuolen lisäksi myös julkisessa terveydenhuollossa.
Johtopäätökset
Yleishammaslääkärin vastaanotolla annettavan parodontologisen hoidon tehokkuutta kuvaavan näytön puute hankaloittaa vertailuja eri maiden välillä. Useassa Pohjoismaassa on merkkejä siitä, ettei parodontologisen hoidon tarpeeseen ole vastattu riittävästi. Suosituksena on, että parodontologisen hoidon tuloksia seurattaisiin koordinoidusti, jotta nykyisten hoitojärjestelmien tehokkuutta voitaisiin arvioida ja tulevaa hoitoa suunnitella entistä paremmin.

The organization of periodontal care in the Nordic countries
Background: Describing and comparing the organization of periodontal care across the Nordic countries may inspire healthcare planners and health politicians in the ongoing process of developing efficient oral healthcare systems. One comprehensive measure as to how cost-effective a periodontal care system is in relation to meeting the periodontal care needs of the population does not exist.
Methods: A systematic literature search was not performed, but the knowledge of each of the authors of the most recent research within the topic was used to provide a descriptive overview of the topic of the organization of periodontal care.
Results: All Nordic countries have periodontal care organized within private and public sectors, providing evidence based periodontal care with variations in the level of public reimbursements for periodontal care patient expenses from 0% to 70%. Expanded care is available for patients with special needs by means of special subsidy schemes and access to public dental clinics as a supplement to the private dental clinics.
Conclusions: Lack of evidence of general dental practice periodontal care efficiency hinders cross-country comparisons. Indications of unmet periodontal care needs are present in several Nordic countries. Coordinated monitoring of periodontal care outcomes are recommended in order to better evaluate the efficiency of existing periodontal care systems and plan for future care.
