Vuoden kirjoitusAlveoliitin hoidon kirjavuus

Alveoliitin hoidosta ei ole olemassa Suomessa yhtenäistä linjaa. Tuoreen Cochrane-katsauksen mukaan alveoliitin hoidosta ei ole riittävästi sellaista tutkimusnäyttöä, joka puoltaisi jotain tiettyä hoitomenetelmää. Alveoliittia voidaan hoitaa ainakin neljällä eri tavalla: 1) joko ei tehdä mitään tai 2) huuhdellaan ja tamponoidaan tai 3) puudutetaan, kaavitaan, huuhdellaan ja tamponoidaan tai 4) puudutetaan, huuhdellaan ja kaapimalla luodaan uusi verihyytymä.

Tämän katsauksen tarkoituksena on tuoda esille tamponoinnissa käytettyjen lääkeaineiden etuja ja haittoja. Gelatiinin ja selluloosan käyttöä tamponina pitää myös arvioida uudelleen.

Alveoliitin hoidosta ei ole olemassa sellaista tutkimusta, joka osoittaisi yksiselitteisesti parhaan hoitomenetelmän. Paremman tiedon puuttuessa rasvasideharsoon lisätty jodoformi ja rasvasideharsoon tai steriiliin taitokseen lisätty Alvogyl-pasta ovat suositeltavimmat. Kaikkein paras ja biologisesti sopivin aine on kuitenkin oma verihyytymä. Sen muodostumiseen ja pysymiseen poistokuopassa tulee kiinnittää erityistä huomiota.

Diversity of treatment methods of alveolitis

In Finland there is no common recommendation for treatment of alveolitis. According to a Cochrane review, there is insufficient evidence on which  to base a recommendation for the best treatment method.

Alveolitis may be treated in at least four different ways:

  1. do nothing except prescribe adequate pain killers,
  2. irrigate with saline and place a dressing,
  3. anesthetize locally, curettage, irrigate with saline and place a dressing, and
  4. anesthetize locally, irrigate with saline and by curettage create a new blood clot.

The aim of this review is to present the pros and cons of different treatments and materials. Use of absorbent hemostats, such as Gelfoam®, Spongostan® or Surgicel®, as  dressing materials is not recommended.

In Finland, it has been common practice to soak the dressing with iodoform together with Chlumsky solution, which is a mixture of camphor, phenol and ethanol. Due to several adverse effects of phenol, it may no longer be a good choice.

In conclusion, no one method is superior to the others, and therefore the optimal treatment still seems to be a normal blood clot.

Because of the diversity of treatment methods and medications used, this topic should be studied further.
